Download, print, complete, and sign the Application for Driver License or Non-Driver ID Card (Form MV-44) (be sure to respond to all of the questions on the back of the form).In order to apply for a replacement by mail, take the following steps: The DMV doesn't allow you to convert a regular document to an enhanced document by mail. If you plan to upgrade to a NY REAL ID upon replacement, you'll need to do so in person and follow the steps for a first-time REAL ID application. NOTE: The fee will not be waived for a replacement of a non-driver photo ID card. (See below for steps on applying in person.)
